    Importation Information on Clearances:
    *Current employees can find their Clearance Dates in their Employee Portal under "Employee Demographics" on
      the right side under residency. Employee clearances on file need dated within 5 years.
     *It is the employee's responsibility to ensure there is not a lapse in valid clearances.
      Lapse in clearances can result in unpaid leave.
    *Only COPIES of clearances should be sent to HR, not originals. Employees should keep originals for their
    *The district is NOT automatically sent clearances from any institution. It is a transaction between the employee 
      and the entity. 

    The following clearances are required of any employee of the Bellefonte Area School District throughout the duration of employment.

    PA Criminal Record Check (Act 34):
         *Online Application: https://epatch.state.pa.us 
           -Select "Submit a New Record Check" and complete.
           -Follow instructions through to the Certification Page.
           -Once the Record Check Request Results appears, click on the blue control number
            (retain control number for your records).
           -Click on "Certification Page" and print.
    **This clearance is due every 5 years

    PA Child Abuse History Clearance (Act 151):
         *Paper Form to mail in: ACT 151 PA CHILD ABUSE APPLICATION 
         *Online Applicationhttp://www.compass.state.pa.us/CWIS/public/home  
           -Create an account or log in if you already have an account.
           -Complete clearance request application.
           -Once application is complete, they will send an email. Follow the instruction to print your certificate.
             Your KEYSTONE ID was issued by the Commonwealth of PA when you initially applied for the first
             PA Child Abuse clearance. Search the email you would have used for "Keystone ID".
             If you cannot find your Keystone ID, Contact the CWIS Support Center at 1-877-343-0494.
    **This clearance is due every 5 years

    Federal Criminal History Record (Act 114):
         *Online Application: https://www.identogo.com/locations/pennsylvania
           -Click on "Digital Fingerprinting".
           -Enter Service Code: 1KG6XN (FBI clearance for PDE-School Districts); For Volunteer use 1KG6Y3.
           -Select "Schedule or Manage an Appointment".
           -Complete your information.
           -Be sure to register for the PDE -School Districts (PA Dept of Education).
     **If you fail to register through PDE Link, you will have to repay to do it again.**
           -Print registration receipt to take with you to fingerprint site.
        *Within a day or two you should receive an email of your "unofficial" report.
          Please send a copy of the report to Human Resources.
          PLEASE NOTE-The link to this report is only good for a short time.
                                 It is also only able to be accessed ONE time.
                                 Please be sure to download and/or print for your records.

    As of July 2, 2020, the only Centre County fingerprint site is:
    1951 Pine Hall Road
    State College, PA 16801
    (Additional locations are available in surrounding counties) 
    *On the day of scheduled fingerprinting, all applicants must have:

         -Valid driver's license or approved photo ID
         -Registration confirmation
         -Form of payment
    **This clearance is due every 5 years.

    Mandated Reporter (Act 126):
         *Online Application: https://bellefonte-pa.safeschools.com/training/home
          -Enter your user name (typically first initial last name). Email lelder@basd.net if not working.

          -Complete PA Act 126 Part 1 and 2.
           Print both certificates and send to 
    Leslie Elder at Central Office.
           If completing this training on a professional learning day, support staff needs to send a copy of the
           Support Staff Training Certificate to Misty Rider in the Central Office.

        **This training is due every 5 years.

    Understanding Youth Suicide and Prevention (Act 71):
         *Online Application: https://my.eduplanet21.com/eduplanet21/
           -For a video tutorial on setting up an account, please click HERE.

           (You must be logged in to the district website in order to view. This video is for current employees only.) 
          -If you have not created an account previously, click on “Create an Account”.
          -Enter your district email address and the Membership code provided in the video link.
          -You will receive an email with the subject “Activate Your Eduplanet21 Account”.
          -Click on the “Activate Account” link in the email. Complete the requested Registration
           information and “Submit”. Enter login information and “Login”.
          -Under “My Learning Paths” click on “Understanding Youth Suicide and Prevention Act 71”.
          -Follow prompts to complete training.
          -Print certificate and send to Leslie Elder at Central Office.
           If you complete this training on a professional learning day, support staff needs to send a copy of the
           Support Staff Training Certificate to Misty Rider in the Central Office.

        **This training is due every 5 years. 

    Arrest/Conviction Report and Certification Form (Act 24):
         *Required upon hiring as well as within 72 hours of any applicable arrest during the duration of employment.
         -Act 24 Form

    Sexual Misconduct/Abuse Certification Form (Act 168):
         *Required upon hiring.
         -Act 168 Form 

    Title IX Training: 
         *All employees are required to complete this training. 
          -Leslie Elder will send login information upon hire. 
         **Training is done one time.

    Act 48 Hours:
         *These hours are not maintained in the Employee Portal system.

           Leslie Elder tracks directly through TIMS. Please disregard this field.
