New Student Enrollment

The following is to be presented when enrolling a NEW student:

  • Birth Certificate

  • Health Immunization Record

  • Parent's/Guardian's Proof of Residency (this includes a mortgage or lease agreement, utility bill, or driver's license showing your address within the District)

  • Custody Documentation (if applicable)

  • Copy of student's Special Education IEP (if applicable)

 

Returning Student Enrollment

The following is to be presented when enrolling a returning student:

Please let us know if your child was previously enrolled in the Bellefonte Area School District so that we may retrieve the past academic file.

  • Birth Certificate (unless this is included in the child's previous academic file)

  • Health Immunization Record

  • Parent's/Guardian's Proof of Residency (this includes a mortgage or lease agreement, utility bill, or driver's license)

  • Custody Documentation (if applicable)

  • Copy of Special Education IEP (if applicable)
